I/The under-18 consent to take part in activities at Little Subs (the Company). In providing my consent I declare that: - I understand that activities are not completely free from risk - I accept that while monitors will take the appropriate precautions to prevent accidents, this can never be guaranteed. I/The under 18 named above is physically fit and healthy and I agree to advise Little Subs of any change in this regard I give consent that, in the event of any illness/accident, any necessary treatment can be administered to me/any under 18's with me I confirm that I agree to abide by the Safety Rules as laid out by Little Subs and to follow the instructions of the staff at all times. I acknowledge that I am responsible for my own safety (and the safety of my possessions) while undertaking any activities at Little Subs I certify that, to the best of my knowledge, I do not have a medical condition or pre-existing injury that might have the effect of making it more likely that I will be involved in an incident that could result in injury to myself or others. To the best of my knowledge I am not pregnant. I acknowledge and accept that the Activities require a moderate level of fitness and can be physically testing and I/the under 18 should not undertake the Activities unless I/they are physically able to. I acknowledge and accept that the Activities are dangerous and there is a risk of personal injury when undertaking such Activities In the event of an accident, or loss or damage to any personal effects, I acknowledge that Little Subs will not be liable for any direct or indirect loss, damage or injury arising from or in connection with the Activities (except for death or personal injury caused by the Company’s negligence). I acknowledge and accept that CCTV images of all park entrants will be monitored and recorded throughout the premises for the purposes of crime prevention and public safety I acknowledge that I and my child/chidren (if applicable) will adhere to the safety rules and advice provided by the Company prior to participation in relation to the Activities and that I have read and fully understand the above, including the associated risks prior to my signing below. I am the parent/guardian of the child/chidren as per booking who is/are under 18 years of age. I wish that/those child/chidren to participate in indoor activities (collectively hereinafter called the Activities ) organised by Little Subs on the booking date of this booking. I declare that if I am not the parent or guardian of the child/chidren I have authority from the child/chidren's parent or guardian to accept these terms. I agree that I am responsible for the child/chidren in my care and undertake to ensure that I/they undertake activities in accordance with the written safety rules and advice that I/they have received on check-in, together with any oral instructions or advice given to me and/or them prior to or during the session. In addition I agree to ensure the child/children wear(s) non-slip socks whilst participating in the Activities. I acknowledge that I am responsible for the safe supervision of the child/chidren as per the booking (and the safety of our possessions). I will ensure that I pay particular attention to any under 12 year olds and will supervise them at all times. I certify that to the best of my knowledge the child/chidren do not have any medical condition (including pregnancy) which might have the effect of making it more likely that he/she/they be involved in an incident which could result in injury to him/her/ themselves or others (if in any doubt, please check with the child/chidren’s parent or guardian.) All participants agree that they shall not participate in any activities provided by Little Subs unless they will comply with these rules at all times whilst on the premises.
